Thank you so much, Dr. Topol, for reading and amplifying the essay. More than anything, I hope it encourages all of us in medicine to pause and reconsider when the clinical picture and the working diagnosis no longer align — before diagnostic inertia and unchecked assumptions take hold.

"One by one, the physicians who followed inherited the error and passed it along like an heirloom."

I read those words - along with all of the others - painfully. The settled conviction by one physician regarding the failure of other physicians. But in this matter the critiquing physician happens to be the brilliant youngest daughter, still grieving her beloved father - Herbert Nelson.

The writing is exquisite; revealing and persuasive. It would have been enjoyable were it not telling such a troubling story: Dr. Lori Nelson's dad, Herb Nelson, died unnecessarily because other doctors became wed to a faulty presumption.

My heart breaks for Lori, and for her sister Rhonda, and for Herbie's wife Lilly, too. Herbert Nelson was one of the five finest human beings that I have had the privilege of meeting, and losing him creates an irreplaceable absence.

But I pray that their loss will be balanced by a justice that, at minimum, leads to the correction of the system that condemned my brother-in-law to die. But even more, I pray that their grief is overwhelmed by loving memories, and eclipsed by a Blessed Hope.
